Estrogen and progesterone do not decline in a neat, predictable line, and progesterone often falls first, years before anyone mentions menopause. Here is how that works:

A comprehensive initial consultation

We take a full history. Symptoms, cycles, sleep, stress, nutrition, medications, previous investigations, family history, and what you want your life to look like.  This is not a ten-minute appointment, and it is conducted by the nurse practitioner directly.

We piece the results together and explain what they mean, so you understand what is happening in your own body. The goal is not to chase one number. It is to find the root cause, and to make sure you leave knowing why you have been feeling the way you have.

 

Comprehensive bloodwork

We look beyond a single blood marker and a standard panel. Depending on your presentation, this may include estrogen, progesterone, testosterone, DHEA, full thyroid function, metabolic and insulin markers, cortisol, inflammatory markers, iron studies, genetic markers and nutrient status.  Much of this testing is covered by OHIP.

An individualized plan

If hormone therapy is appropriate, we discuss the options, the evidence, and the risks and benefits in the context of your own health history and what you can reasonably expect. If something else is driving your symptoms, we tell you that and we treat what is actually there and the root cause.

Ongoing follow up and adjustment

Hormone therapy is not set and forget.  We monitor how you respond, we retest, and we adjust. Getting the dose and the delivery right for you is an ongoing process, not a single prescription.

Is bioidentical hormone therapy safe?

Like any medical therapy, hormone therapy has risks and benefits that depend on your individual health history, your age, the type of hormone, the dose, and the route of delivery. The evidence has changed considerably in the last two decades. We discuss the current evidence and your specific circumstances openly so you can make an informed decision.

Is this covered by OHIP?

Consultations at NOW Health Clinic are not covered by OHIP. However, much of the bloodwork and diagnostic testing we order is OHIP covered. Some private insurance plans cover nurse practitioner visits under a Health Spending Account. We are transparent about fees before you book.

Do I need a referral?

No referral is required. You can book directly. Care begins with a meet and greet so we can understand what you are looking for and make sure we are the right fit for you.

How long before I feel a difference?

It varies by person and by symptom. Some symptoms, such as sleep and hot flashes, can respond within weeks. Others, such as body composition, take longer. We monitor how you respond, we retest, and we adjust rather than expecting the first dose to be right.

Do you treat men as well as women?

Yes. We provide hormone assessment and treatment for both men and women, including testosterone replacement therapy for both. Testosterone in women is frequently overlooked, despite its role in energy, mood, libido, muscle mass, and cognition. In men, low testosterone is often attributed to stress or ageing and rarely investigated properly.

What testing is involved?

Comprehensive bloodwork, tailored to your presentation. Depending on your symptoms this may include estrogen, progesterone, testosterone and DHEA, full thyroid function, cortisol, insulin and metabolic markers, inflammatory markers, iron studies, and nutrient status. We go beyond a basic panel because a normal result on a narrow test does not mean your hormones are optimal.

Does hormone therapy cause breast cancer?

This is the question we are asked most, and it deserves a real answer rather than a reassurance.

Much of the fear traces back to a single study published in 2002, which was widely reported and widely misunderstood. The women studied were, on average, well past menopause, and the results were applied to all women regardless of age, timing, or type of hormone. Since then, the evidence has been re examined extensively. Risk is not one number. It depends on your age, how long since menopause, which hormones, which dose, and which route of delivery, and on your own health and family history.

For many women, the risks are smaller than they have been led to believe, and the benefits are greater. For some women, hormone therapy is not appropriate. That is exactly what an assessment is for. We go through the evidence and your own history with you, honestly, and you decide.

What does this cost, and what am I paying for?

Consultations are not covered by OHIP. Much of the bloodwork and diagnostic testing we order is. Some private insurance plans cover practitioner visits under a Health Spending Account.

What you are paying for is time and depth. A full history rather than ten minutes. Comprehensive testing rather than a single panel. A practitioner who assesses estrogen, progesterone, testosterone, thyroid, cortisol, insulin, and nutrient status together rather than in isolation. And ongoing follow up, because getting hormone therapy right takes adjustment.

We are transparent about fees before you book. There are no hidden fees.

Do I have to stay on it forever?

No. Hormone therapy is a decision you revisit, not a life sentence. Some people stay on it for years because they feel well and the benefits continue. Others use it through a particular window and come off. We review it with you regularly, and stopping is always an option we discuss openly.

Will hormone therapy make me gain weight or lose weight?

Hormone therapy is not a weight loss treatment, and we will not sell it to you as one.

What we can say is that hormonal change affects where the body stores fat, how it handles blood sugar, how well you sleep, and how much muscle you hold. Many people find that when those things are addressed properly, weight becomes easier to manage. But weight is driven by many factors, and we assess metabolic health, insulin, thyroid, and lifestyle alongside hormones rather than treating hormones as the whole answer.

What forms does hormone therapy come in?

We offer oral, creams, patches, suppositories, and injectable hormones. Which is right for you depends on your physiology, your symptoms, your health history, and your life. Route of delivery is not a small detail, it affects how the hormone behaves in your body, and it is part of what we discuss with you rather than deciding for you.
